I received an invoice from Quest Diagnostics. What should I do?

If you placed your order through Accesa Labs and provided the Quest location with our lab order but received an invoice, please follow one of these steps:

1) Email us at http://support.accesalabs.com/customer/portal/emails/new. Either scan or take a picture of your invoice and attach it to your email.

2) Fax your Quest invoice to (310) 606-2688. Please provide an email address or phone number so we can contact you once we investigate your invoice.
